To receive your cash prize, download the W-9 Form and Winner Release. Then fill out both documents and fax them – WITH COPY OF GEORGIA DRIVER'S LICENSE – to 404-897-6495 (ATTN: Buffy Berman) or email buffy.berman@coxinc.com. Once everything has been processed by our Business Office, your check will be mailed within 4-5 weeks.
NOTE: No check will be issued without a signed W-9 Form, signed Winner Release, and copy of driver's license on file. All paperwork must be received within 30 days of winning or prize will be forfeit.
